Report a Lost, Stolen or Damaged Passport

What to do:

  • Please notify the nearest N.Z. Police station of the loss/theft.
  • Inform the U.S. Consulate of the loss/theft by telephone at: +64 9 303 2724 ext. 2800.  Leave a message on our voice mail if necessary.

If you decide not to apply for a new passport immediately, you may report your lost or stolen passport by completing Form DS-64 and mailing it to: U.S. Consulate, American Citizens Services, Level 3 Citigroup Center, 23 Customs Street East, AUCKLAND

To obtain a new passport, you must apply in person by appointment to the Auckland Consulate at Level 3, Citigroup Center, 23 Customs Street East, Auckland (cnr. Commerce St) or at a Consular Outreach visit.  Note: An appointment is needed for this service.

You will need to:

  1. Complete Form DS-11, Application for Passport. Please note that some of the instructions apply only to U.S. citizens residing in the U.S.
  2. Present Proof of U.S. Citizenship.
  3. Present Proof of Identity.
  4. Provide one Passport Photo (Photos must be 2″x2″ in size with a background that is uniform, plain and white, and free of shadows). Eyeglasses must not be worn in a passport photo.  Effective for applications filed on or after November 1, 2016, eyeglasses must not be worn in a photo for a passport application, except in rare circumstances when eyeglasses cannot be removed for medical reasons, e.g. the applicant has recently had ocular surgery and the eyeglasses are necessary to protect the applicant’s eyes. In such a case, a medical statement signed by a medical professional must be provided.  Submitted passport photos must have  a white, off-white or cream background. Thank you for your compliance.***
  5. Pay the Applicable Fee.
  6. Provide a Social Security Number.
  7. Complete a Statement Regarding Lost Or Stolen Passport, DS-64.
  8. Provide a courier envelope for your passport to be mailed to you.
  9. Make an appointment (NOTE below about appointments).
  10. Applicants must bring a photocopy of the documents they are submitting in support of their applications.

Appointments

Please remember to print your appointment confirmation and bring it with you to the Consulate.

Please be advised that the appointment time is a guide as to when you should plan to be at the Consulate. It is not necessarily the time you will be seen by the Consular Officer. Applicants should plan to spend at least an hour at the Consulate.

Access to the U.S. Consulate Auckland

For the safety of all of our customers and staff, all visitors to the Consulate must pass through security screening at our reception area before entering the Consulate. Please allow time for screening before your appointment.

The Consulate General’s hours of operation are from 8:00 AM – 12:00 PM and 1:00 PM – 4:00 PM.  You will not be able to access the Consulate’s reception area outside of these hours.  At no time are you authorized to remain in the lobby of the Citigroup Building while awaiting Consular services. We appreciate your patience and cooperation.

Please do not bring luggage, including backpacks, to your appointment. You will not be able to bring these items into the Consulate, and there is no storage available in our office.

Mobile telephones and other telecommunication devices, electronic recording devices, and cameras may not be taken into the Consulate. Please turn them off, and leave them with the guards at the reception area. They can be collected after your appointment.

Aerosols of any kind are prohibited, and must be left with the guards until after your appointment.

Any type of headwear, including baseball caps, must be removed while in the Consulate.
